Deceased Notification Form

If the borrower is deceased, the federal student loan may be discharged. In the case of a Parent PLUS loan, the loan may be discharged if the responsible party or the student is deceased. If the death certificate is not available, you can provide the city, state, and the last name of the deceased at time of death.
